Application
requirements have changed as of April, 2008. Please contact a
SOMB staff member for more information about these changes. New
application forms will posted soon. To request a fingerprint card,
please contact Charla Phagan at Charla.Phagan@cdps.state.co.us
There
is $100 application fee along with the $39.50 fee for fingerprint
card processing. The SOMB only accepts Money Orders or Certified
Cashier's Checks. The $100 application fee should be made out
to the Colorado Department of Public Safety. The $39.50 finger
print card processing fee should be made out to C.B.I.

We collected frequently
asked questions as we conducted training across the state on the
Management of Juveniles Who Have Committed Sexual Offenses.
Q1: If a provider is listed to work with adults
does that automatically make them a listed provider for juvenile
clients?
A: No. Providers must make seperate applications
to work with juveniles and/or adults.
Q2: When will the Juvenile Standards go into
effect?
A:The Juvenile Standards were published in 2002
and each year the Board has issued an Implementation Statement
to assist providers and community members in understanding the
implementation process. Listed providers are expected to comply
with the Standards. Please go to the top of this page and click
on "Board
Position on Juvenile Standards".
Q3: Who pays for informed supervision once a
juvenile who has committed a sexual offense returns home?
A: The position is not paid. Informed supervisors
are parents, caregivers, professional personnel involved in the
evaluation, treatment, supervision and management of a juvenile
who has committed a sexual offense. Informed supervision also
occurs at schools and other locations where the juvenile who has
committed a sexual offense might be involved in any activity.
It is the responsibility of the multidisciplinary team to identify
informed supervisors and educate them to the level needed for
effective informed supervision in a specific setting.
Q4: If I am a Domestic Violence Management Board
listed provider who has already submitted my fingerprints as required
by the Domestic Violence Board, do I need to have my fingerprints
re-done?
A: Good news!! No, you do not have to have your
fingerprints re-done, you DO need to indicate on your application
that you have submitted them in compliance with the Domestic Violence
Board's stipulations.
Q5: Are there specific tests that are required
in an evaluation?
A: No, there ARE specific areas that must be
evaluated as outlined in Standard 2.500. Please see the matrix
beginning on Page 30 of the Standards regarding evaluation procedures.
Q6: Who
should fill out the 2nd or Subsequent Re-application?
A:
Any treatment provider, or evaluator at the Full
Operating Level, who has been listed and has already re-applied
once for their listing can fill out this application. This application
is for anyone that is entering into their second re-application
period and any period thereafter.
Please note that any individual who is eligible to fill out this
application have different criteria to fulfill their listing status.
Please refer to Section 4. 420 in the Adult Standards and Guidelines
for the Assessment, Evaluation, Treatment, Monitoring and Behavioral
Monitoring of Adult Sex Offenders and Section 4.620 in the Juvenile
Standards and Guidelines for the Evaluation, Treatment, and Supervision
of Juveniles Who Have Committed Sexual Offenses for specific criteria
for this application.
