APPLICATION MATERIALS

Application requirements have changed as of April, 2008. Please contact a SOMB staff member for more information about these changes. New application forms will posted soon. To request a fingerprint card, please contact Charla Phagan at Charla.Phagan@cdps.state.co.us.

There is $100 application fee along with the $39.50 fee for fingerprint card processing. The SOMB only accepts Money Orders or Certified Cashier's Checks. The $100 application fee should be made out to the Colorado Department of Public Safety. The $39.50 finger print card processing fee should be made out to C.B.I.
